All students in SJCDS must read and sign the student conduct code. There is an optional section that must be completed and returned to your child's teacher before your child may bring their laptop computer or personal electronic devices to school. AUP Signature and Personal Electronic Property Waiver Form 2012-2013 This form is required only if you do not wish to have student directory information released. Student Directory Op-Out Form 2012-2013 The Emergency Card information is very important.  Please be sure that all contact information is current. This is a yellow card that will come home with the first day folders. Remember, a form must be completed and returned to the school office before your child can receive prescription or over the counter medication at school.
